The CLIDE Files
How custom-tailored care keeps patients in their contacts.

A 47-year-old woman had successfully worn contact lenses for 35 years, but now complained of dryness that reduced her wearing time from full waking time to 5 hours or less. Even with reduced wear, she felt uncomfortable. Slit lamp exam revealed 3-9 staining on the periphery of the cornea.

After ruling out other possible causes, her optometrist, Frank D. Fontana of St. Louis, realized this patient had contact lens-induced dry eye, or CLIDE. For him, the question became: How do I keep this patient in contact lenses?

You may be asking that question repeatedly in your own practice, especially when you consider that about half of contact lens wearers report dry eye symptoms.1 The problem is likely to become more prevalent as the population ages and more presbyopes opt for contact lenses. Finding a solution for them is crucial because dryness-related discomfort is one of the main reasons patients discontinue contact lens wear.2-4

Still, this doesn’t mean patients with dry eyes cannot wear contacts. For some, managing symptoms of CLIDE is as simple as instilling artificial tears, practicing lid hygiene or removing and rehydrating lenses every few hours. For others, a more comprehensive—and sometimes creative—approach is in order. That’s what these cases illustrate.

Case 1: A New Material Girl

The previously mentioned patient had worn FluoroPerm 30 (fluorosilicone acrylate) lenses comfortably for many years. Although Dr. Fontana could have switched her to a soft toric lens—the patient had 1.75D of astigmatism—he wanted to keep her in an RGP. She was accustomed to the crisp vision an RGP offers.

Dr. Fontana switched the patient to Trans-Aire (silicone acrylate) lenses. She no longer complains of dryness, and she has increased her wearing time. The 3-9 staining disappeared.

Discussion. When patients present with CLIDE, a change in lens material may be in order. However, you must first confirm that the patient truly has dry eye. “Just because the patient has dry eye symp- toms doesn’t mean he or she has dry eye,” says Scranton, Pa., optometrist Joseph P. Shovlin. An ocular allergy may cause the patient to experience dry eye-like symptoms such as itching and burning, even though this is not true dry eye. Not only that, but the topical antihistamines used to treat ocular allergies will worsen a dry eye, adds optometrist Alan G. Kabat of Nova Southeastern University College of Optometry in Fort Lauderdale, Fla.

Your diagnosis starts with a thorough history. Patients with dry eye usually present with subjective complaints such as burning and itching, foreign body sensation, redness, photophobia and stringy mucus. However, patients with CLIDE are more likely to present with foreign body sensation rather than burning or itching as their primary complaint, Dr. Kabat says.

Dr. Shovlin suggests using a dry eye questionnaire developed by C.W. McMonnies.5 This questionnaire asks about primary and secondary symptoms; risk factors including age, sex, contact lens wear and medications; and systemic and ocular factors that may be associated with dry eye syndromes.
Optometrist Kristy Canavan, also of Nova Southeastern, suggests you consider the type of lenses the patient is wearing (daily or extended wear; low or high water content). Also ask the following: Does the patient comply with the recommended replacement schedule? What care system does the patient use? Does she enzyme regularly to remove protein deposits? Does the dryness disappear after she removes her lenses?

Dryness may result from a lens that doesn’t wet well and may be secondary to oxygen deprivation. In such instances, you need a lens surface with both improved wettability and higher oxygen permeability. For Dr. Fontana’s patient, the Trans-Aire material offered both.

Although fluorine is effective at resisting protein deposits, it tends to attract lipids, says Mike Johnson, director of consultation services for Art Optical. So, patients with a higher lipid content in their tear consistency may experience buildup on lenses. The Trans-Aire material, a silicone acrylate, may help prevent these types of deposits.

Case 2: Watered Down

Optometrist Janice M. Jurkus of Illinois College of Optometry fit a new contact lens patient with Optima, a thin, 38.6% water content lens. At follow-up, however, the patient complained of dryness whenever she wore the lenses, despite frequent use of artificial tears.
Dr. Jurkus switched her to Surevue (etafilcon A), a thicker, 58% water content lens. The patient reported that she was able to wear the lenses longer with decreased use of artificial tears and improved comfort.

Discussion. Keep in mind that every soft lens absorbs moisture from the surface of the eye, Dr. Kabat says. Lenses with higher water content have greater dehydration, which also reduces oxygen transmissibility. So, clinicians often consider these lenses less than ideal for patients with marginal dry eye. (Of course, there are exceptions such as the Proclear omafilcon lenses and Extreme H2O, both of which claim less evaporation even with a 59% water content.)

As this case illustrates, however, you sometimes must take the opposite approach and go with the higher water content.

Dr. Jurkus did a pilot study of 10 subjects in 1995 and found that marginal dry eye patients can wear high-water content, disposable soft lenses on a daily wear basis. However, she found that such patients more readily accepted a thinner, high water content lens design more readily than a thicker design in the same material.6 A more recent study of 20 soft lens wearers found no correlation between lens dehydration and subjective dryness and comfort.7

When increasing the lens thickness is not enough, you might consider ordering a prism ballasted lens. This provides a reservoir of sorts for hydration, Dr. Shovlin says.

Case 3: Withdrawing The Deposits

A patient in her late 40s, a long-term wearer of FluoroPerm 30 lenses, reported a heavy coating on her lenses. Dr. Jurkus initially prescribed artificial tears, but the coating reappeared within 10 minutes of instillation.

Dr. Jurkus next added SupraClens daily liquid protein remover to the patient’s care regimen. The protein deposits dissipated, and the patient’s comfort increased.

Discussion. With some patients, simply modifying the care regimen may solve the problem without having to switch lens materials. In this particular case, the product’s ease of use helped encourage patient compliance.

As this patient aged, her aqueous production declined, hence the protein deposits on her lenses. You’ll need to educate patients that age-related changes may affect comfort, even in those who have worn contact lenses successfully for many years. “Although we cannot cure this patient’s dry eye, we eliminated its major side effect so that she could continue to wear contact lenses,” Dr. Jurkus says.

Case 4: Giving a Plug For Punctal Occlusion

A 47-year-old female presented to Dr. Canavan at Nova Southeastern University complaining of dryness when she wore her contact lenses. Dr. Canavan switched her from Acuvue (etafilcon A, 58% water content) to Proclear (omafilcon A, 59% water content) and then PureVision (balafilcon A, 36% water content) lenses, but she experienced minimal relief. Punctal plugs eventually solved this patient’s problem.

Discussion. If you’ve treated or removed underlying factors, prescribed artificial tears, and changed lens materials and/or care regimens, you may then need to go to punctal occlusion.

Had this patient’s problem been the result of instability of the lipid layer, Dr. Canavan says, she might then have started the patient on lid scrubs and warm compresses. She also would have changed material and reduced the patient’s wear schedule. Dr. Canavan determined that this patient’s problem was caused by aqueous deficiency as opposed to tear film instability. So, approaches such as lid scrubs and warm compresses would not have alleviated the problem.

Studies have confirmed that punctal occlusion can provide a long-term solution to keeping a patient in contact lenses.8 For example, researchers from Indiana University School of Optometry found that intracanalicular silicone plugs improved the signs and symptoms of dryness, although the degree of improvement decreased somewhat over time.9

The disadvantages to punctal occlusion may include epiphora and irritation from a dislodged plug. Others have experienced corneal and conjunctival abrasions, and some clinicians questions their effectiveness in contact lens patients with dry eyes.10

Case 5: Stepping Up to Help A Keratoconus Patient

A 51-year-old female with keratoconus experienced significant dryness with fluorescein staining of the cornea and conjunctiva. The dryness made it difficult for her to wear her rigid contact lenses. Given that she had worn lenses for several years with no problems, Dr. Shovlin ruled out ocular allergy. Instead, he believed the dryness likely occurred post-menopause.

Because of the patient’s visual demands, Dr. Shovlin needed to keep her in an RGP lens, which is usually effective for most patients with dry-eye symptoms. “But we had to make sure the lenses were as clean as we could possibly keep them,” he adds. So, he instructed her to use warm compresses and lid scrubs, and instill artificial tears. It was also important to make the lens edges as thin as possible and make sure the patient meticulously cleaned, disinfected and lubricated her lenses.

Dr. Shovlin also occluded the patient’s inferior puncta with silicone plugs. The patient reported increased lens comfort and wearing time, but then experienced spontaneous extrusion of the plugs. Thermal cauterization of the superior and inferior puncta minimized the problem.

Discussion. By using “step-up therapies,” Dr. Shovlin was able to keep this patient in her RGP lenses. One pearl he offers: Don’t forget simple measures such as lid hygiene. When these measures alone failed, punctal occlusion was able to abate her symptoms dramatically. Remember to also look for concomitant allergy, especially in kerato- conus patients, Dr. Shovlin adds.

Not every dry eye patient will be able to continue wearing contact lenses no matter how hard you try. You’ll need to abandon contact lens wear if other approaches don’t work, if the patient develops chronic secondary infections or the potential for serious visual compro- mise, or if the patient is grossly negligent. With enough persistence, however, you may be able to keep many other patients in their contact lenses and make successful long-term wearers out of new patients.

Salt Allergy and Bloating
The average person eats at least 3 times as much salt as they should - and not because of their salt shaker. The processed foods, canned foods and chips that the civilized diet includes are all incredibly oversalted. By changing your diet to one of fresh food, fresh meat and low junk food, you naturally consume your proper amount of salt. Salt can cause huge amounts of bloating - especially around the belly - so having that salt removed from your diet can remove a major source of bloating.

Decreasing sodium is one of the easiest changes you can make in your diet. Once you start a low-sodium diet, your tongue becomes more sensitive to the salt content in foods. Eventually you will not notice the lack of salt. The change takes about 30 days for most people. Use natural spices like oregano and rosemary to add flavor, but beware of seasonings with hidden salt, such as Cajun seasoning and blackening spices.

Yah anyway thinking about it i think i was pretty mean to G when she ask to lend her my notes... i wanted to teach her a lesson and said no... cos she is late for class every day not just ten minutes but till up to forty minutes before .. you know how important our practicals are right i mean it's what u have to perform on the general public next time... how can you be late all the time and dun seem bothered by it?

If i lend her my notes... is she just going to take things for granted and be late always? i was a bit i dunno pissed by her hack care attitute and of course i did not scold her.. who was i to do that?

i just sort of advised her maybe she should try to turn up early... and today she came earlier was kinda glad that my advice had some effect.. anyway thinking about it.... i just msg her in msn asking her if she need the notes and i will lend it to her after i rewrite it which is dunno when. I guess i have been pretty slacky lately. I think she needs the notes. And i guess she must be kinda shocked i offer her the notes.. well whatever i can be an angel all the time it doesnt matter sometimes no one appreciates.. i think i will just do what i feel is right yah? Just pray she wun go back to being the late queen?

Anyway. i am sick of replying my mum each time she nags. I think she knows i am pretty upset? i dunno i am not bah. i just feel blanked out. yah and she keeps asking me to eat now. How to say.. no mood?..i dun know what to reply back. i just stare blanky at the screen. Maybe from today i wun talk at home unless neccessarily. dunno what's the point anyway. i say no i dun want to eat she also go prepare the food. I guess my answer is not that important anyway.

The Lovers' Test



A man (M) and a lady (L) who are very much in love and devoted to one another, are separated by a river with no way of getting across to the other side.

On L's side of the river, there is a boatman (B) who is able to take her over to the other side of the river but refuses to do so unless she pays him a price of $20, twice his normal fare. L has no money.

Another man (S) then tells L that he will give her $20 if she sleeps with him. L agrees to do so and on receiving the $20 pays B, who takes her over to the other side of the river. She is reunited with M and they are very happy together.

However, a friend of M's (F), finds out what L did with S and immediately tells M. On learning the news, M goes to L and ends things with her, stating that he wants nothing more to do with her.

Well... why quit smoking?

You'll have better orgasms

Smoking furs up your arteries, reducing blood flow to your vagina, which can make orgasms harder to achieve. Smoking expert Dr Toby Chai, a urologist from the University of Maryland, US, says: 'No studies have been done in this area yet, but it's a matter of common sense that any decrease in blood flow caused by smoking is likely to result in problems with lubrication and orgasm.'

Want your man to quit, too? Remind him that smoking can cause impotence by cutting blood flow to the penis. Male smokers also have lower levels of testosterone - the hormone necessary for sex drive and sperm production. Luckily, it's reversible: a study of 20 impotent heavy smokers found that within six weeks of quitting, six of them were miraculously cured.





You'll become more fertile

Women who smoke are about 30 per cent less fertile than non-smokers and reach the menopause nearly two years earlier. Smoking affects your sex hormone levels. It may also damage your eggs and cause abnormalities in your Fallopian tubes.

If your man gives up smoking, he'll also improve your chances of conceiving. Many male smokers have low sperm counts, and the sperm they do have are an abnormal shape and less mobile.





Your bad back will ease up

Smoking makes you twice as prone to a slipped disc. This may be because it reduces blood flow to the cartilage discs between your vertebrae, restricting the amount of nutrients and oxygen they receive. Smoker's cough can also cause the pressure inside your discs to rise, which may give you a bad back.




Your hearing will get better

Smokers go deaf on average 16 years earlier than people who've never smoked. Experts aren't sure why, but it may be down to reduced blood flow to the ears or because carbon monoxide in smoke damages nerves in your ears.





Your breath will stop smelling of rotting eggs

Tobacco contains sulphur - the chemical that gives rotting eggs their distinctive pong - with cigars and pipes the worst offenders. So it's no surprise that your breath stinks so much when you smoke.





Cuts will heal quicker

The nicotine in tobacco constricts blood vessels, starving wounds of the oxygen they need to promote healing - hence the length of time it takes all those niggling scratches to heal.




Your skin will look younger

Smokers in their forties have facial wrinkles similar to non-smokers in their sixties, especially around the mouth, eyes and neck. This is because smoking constricts capillaries, thereby reducing your skin's supply of oxygen and other nutrients.

Tobacco smoke dries your skin and changes the shape of your collagen, skin's natural elastic. Smoke also irritates your eyes, making you squint, which may be another reason you get crow's feet.

However, it is reversible - although some experts say your skin will probably look worse for a few weeks after you quit, as your body clears away toxins.



Food will taste and smell better

The 4,000 or so chemicals in cigarettes dull your senses of smell and taste - so unpleasant aromas don't seem so bad and pleasant odours don't seem as nice. Smoking can also contribute to your sense of taste deteriorating significantly as you get older.



You'll stave off colds and throat infections better

Smoking impairs your immune system, which means you suffer from more chest infections, take longer to fight off colds and are more likely to get a post-surgery infection.

i know why i cant slim down le i always tend to comfort eat yah really.... especially when i feel very alone i just eat and eat win liao lor now i am really fat haa haa dun ask me why SY from next class so power heard that she dun eat at all one lol if i can be like her good liao lor...

i wish i can go Paris or somewhere far away... with just a backpack and good shoes nice spagetthi top and nice shorts then walk all over the streets then when i am tired can sit down by a stone start taking out a canvas or something and start to draw the sunset.. that's what i call life man.... =(

Suddenly i just feel so deprived... yah i think my soul is deprived of something fulfilling....haiz i hate it i wish i can dun eat forever at times... just bury me somewhere will you...

Boosting Your Metabolism Naturally for Healthy Weight Loss

Reaching and maintaining a healthy weight, feeling fit, and glowing with healthy body confidence is a high priority for many of us. There are a lucky few who are naturally slender with a rapid metabolism, but for the rest of us these things may not come so easily.

So how can you reach your perfect weight and maintain it throughout your life?

An efficient metabolism is key. The good news is that eating to boost your metabolism not only helps you reach and maintain a healthy weight, but it also fuels you with greater energy and wellness across the board.

What exactly is metabolism?

Elson Haas, M.D., in his book Staying Healthy with Nutrition (Celestial Arts, 1992), defines basal metabolic rate (BMR) as the rate at which your body burns calories to maintain its functions at rest. He explains that your BMR is affected by such characteristics as age, diet, activity level, body fat percentage, body weight and genetics.

Our active tissues, such as muscles, have a higher metabolic rate than inactive tissues, like fat. When your metabolism is running at peak efficiency, it's easier to reach and maintain a healthy weight.

For most of us, a healthy metabolism translates into radiant energy and a cheerful spirit. A sluggish metabolism, on the other hand, is likely to sap our energy, as well as promote weight gain and digestive troubles.

If a healthy metabolism is so important, why don't we all have one?

There are a number of factors that impair your metabolism, such as the "yo-yo" dieting syndrome of repeated weight gain and loss over time.

Dr. Haas explains that when you diet and reduce calorie intake, your body reacts like it would in times of famine, lowering the rate at which you burn calories (your BMR) to conserve your weight. Ironically, this means that the more you diet, which slows your metabolism, the harder it is to lose weight.

So how do you break this cycle?

The answer may surprise you. Research suggests that a primary key to recovering a healthy metabolism is simply to eat more often.

The trick is to eat when you're truly hungry, but to stop before you're stuffed. Doing so maintains healthy blood sugar levels, addresses cravings, and reduces the chance that food will be stored as fat, rather than burned.

So then, which foods are most effective in boosting metabolism and promoting healthy weight loss?

Your best guide to choosing the right foods is the glycemic index. Research from the University of Sydney defines this index as a ranking of foods based on their immediate effect on blood sugar levels.

Carbohydrates, such as refined white breads and pastas, white potatoes, and bakery products like cookies and cakes have the highest glycemic indexes. They break down quickly during digestion, causing a rapid blood sugar response.

Whole, unprocessed carbohydrates—like whole grain breads, brown and wild rice, whole wheat pita bread and tortillas, yams, oatmeal, popcorn, soy products, beans, nuts and seeds—on the other hand, come in low on the glycemic index. They break down more slowly, releasing glucose gradually into the blood stream.

By choosing low glycemic index foods, you avoid the blood sugar swings that send insulin soaring which rapidly turns calories into fat. Not only will whole, unprocessed carbohydrates promote a more gradual energy release than processed and refined foods, these healthier options are also higher in nutrients and fiber.

Increasing your intake of high-fiber carbohydrates over fat is another smart choice, as they are more likely to be used for energy than tucked away in fat storage cells.

What about the way you eat?

Research suggests that consuming smaller meals more often promotes an efficient metabolism and a healthy body weight. Generally speaking, five or six mini-meals consisting of about 250 calories each is a good formula.

Smaller portions enable you to better fit in your five to seven servings of fresh fruits and vegetables. This kind of eating also helps to prevent the excess insulin produced with fewer, heavier meals which encourages your body to store calories as fat.

Our contemporary lifestyle has so greatly slowed our metabolic rate,

many of us tend to store fat more consistently than we burn it for energy, says Pamela Smith, R.D., in her book The Energy Edge (HarperCollins, 1999).

Erratic eating patterns promote a sluggish metabolism in the same way dieting does, because your body is designed to retain calories in times of potential famine to help you survive.

a recent Vanderbilt study showed that overweight breakfast-skippers who began eating breakfast lost an average of 17 pounds in 12 weeks. The research also suggests that those who eat breakfast tend to be less hungry throughout the day, and therefore less impulsive about unhealthy snacking.

What about supplements?

A full-spectrum, multi-vitamin/mineral formula will help supply the many nutrients your body needs for healthy digestion, energy production and all other body processes.

In addition, several other nutritional supplements have been shown to help promote a healthy, active metabolism, and deserve a closer look:


Green tea, a source of powerful antioxidant activity, also appears to help boost metabolism.

A 1999 study in the Journal of Clinical Nutrition showed that sedentary individuals eating a normal calorie diet were able to significantly increase their ability to burn calories when given green tea extract with meals.

Chromium, an essential mineral found in supplements, nutritional yeast, whole grains, and other foods, plays its own important role in healthy metabolism.

In his book The Encyclopedia of Nutritional Supplements (Prima Publishing, 1996), Michael Murray, N.D. also discusses chromium as a weight loss aid. He explains that a primary method for losing weight is to increase the sensitivity of the body's cells to insulin, the hormone that controls hunger and fat metabolism.

By helping insulin work more effectively, chromium enables your body to use more calories to build muscle, storing fewer calories as fat.

Pyruvate—a derivative of pyruvic acid found in red apples, red wine, dark beer and supplements—may also be a useful tool in boosting metabolism and promoting weight loss. Pyruvate works by increasing your cellular "energy factory" so that you burn fats and carbohydrates more efficiently.

Additionally, the amino acid L-carnitine—found in supplements and vegetarian foods such as tempeh, whole wheat, and avocado—has been shown to help burn body fat.

Essential fatty acids (EFAs) are vital to a rapid metabolism and healthy body weight. These fats are referred to as "essential," since we can't manufacture them in our bodies and must get them from foods.

The EFAs consist of omega-6 linoleic acid and omega-3 alpha-linolenic acid. The average American gets plenty of the omega-6 fatty acids, which are found in most nuts, seeds, and vegetable oils, however, the omega-3s are harder to come by. The omega-3 fats are supplied by many vegetarian sources, including flaxseed oil, hemp seeds, pumpkin seeds, walnuts and dark leafy greens.

In fact, flax oil is one of the very best sources, with up to a 60 percent concentration of the essential omega-3s.

Why should you make a point of consuming these important fats?

Dr. Murray points out that a lack of the EFAs can lead to obesity, fatigue, frequent colds and sickness, lack of motivation and nearly 60 other serious health problems.

Ann Louise Gittleman, M.S., C.N.S. discusses in her book Eat Fat, Lose Weight (Keats, 1999) how essential fatty acids promote healthy cardiovascular, immune, reproductive, and central nervous systems, as well as help to regulate energy production and fat metabolism.

These vital fats also stimulate calorie burning in your brown fat tissue, according to a 1997 study in the International Journal of Obesity-Related Metabolic Disorders (Nov;21(11):955-62).

Brown fat is the layer of fat underlying your white fat, and is a strong ally in weight loss because it works to burn excess calories by stimulating the production of heat in the body. White fat, on the other hand, is simply a fat storage site with no metabolic activity.

Keeping ourselves well hydrated with at least 8 glasses of water daily is also important for healthy metabolic function. Consuming water at room temperature is best when exercising, but when you're at rest, drinking ice water will actually increase your metabolism as your system works to warm it to body temperature.

So, how can you bring it all together for results that you can actually measure?

All you need to do is make healthier choices one step at a time. Taking a closer look at diet is a must—not only what we eat, but also how we eat. Trading in large meals and "hit and miss" eating for more frequent, consistent mini-meals will help stabilize your blood sugar and get your metabolism back on track.

Jumpstart Your Metabolism

If you work at it, you can actually increase the rate at which your body burns calories. This is known as metabolism. Many factors affect your metabolic rate.

Metabolism (or total energy expenditure) is basically composed of 3 parts:

(1) Resting Metabolic Rate (RMR) - This is the amount of energy (calories) needed to run the vital functions of your body (i.e. breathing, heart beat). RMR is believed to account for about 65 to 75% of the calories your body burns in a day. As fat content in the body increases, the RMR decreases. Muscle deterioration will also decrease the RMR. To combat a decrease in the rate at which your body burns calories at rest, build more muscle and shrink those fat stores.

(2) Thermic Effect of Food (TEF) - This involves the work associated with digesting the food you eat. The amount of energy used here ranges from 5 to 15%. The composition of your diet will have an effect on this part of metabolism. When the ratio of carbohydrate to fat is high, metabolism is higher. The body has to work much harder to convert carbohydrates to fat than it does dietary fat which is already in a storable form.

(3) Effect of Physical Activity - This is the energy your body burns to perform daily activities, exercise, play, etc. On average, physical activity accounts for 20 to 40% of the calories burned by your body in one day. It is here where you can have the greatest effect on your metabolism. This part of metabolism is considerably lower in a person who is sedentary (inactive) versus a very active person. The intensity of the activity, how long the activity is performed, and the frequency with which the person performs the activity all have an effect on metabolism. This is probably a no-brainer, but the more intense the activity, the more calories are burned; the longer you do the activity, the more calories you are burning; and the more often you do the activity, the more calories are used up.

Unfortunately for us women, we tend to have less muscle mass than men. This is the way we are made, (no, guys, this is not an excuse, just the facts). Thus, women typically have a lower RMR than men. People under the age of 30 usually have a faster metabolic rate for the same reason - higher muscle mass. However, this does not have to hold true. Keep your muscle mass by lifting weights three days a week. Studies have shown that in as little as three months of moderate weight training will boost your RMR.

The Effects of Food on Metabolism

Anything that speeds up the heart rate, such as stimulants, will temporarily increase your RMR. Examples of food stimulants include caffeine and spicy chili peppers (the key being spicy). Complex carbohydrates (i.e. bread, rice, pasta, potatoes, beans, lentils, fruits and vegetables) contain about half the calories by weight of fat. Plus, it is more work for the body to convert carbohydrates to body fat.

Alcohol is a concentrated calorie source and contains few nutrients. It actually reduces the body's ability to burn fat. The soluble fiber in oat products not only lowers cholesterol but enhances weight loss.

Walking within 10 minutes of eating can actually boost your TEF helping your body burn 10% more calories than normal. Eating every 3 to 4 hours (6 to 7 small meals per day) will increase your TEF, thus your metabolism.

Power List of Ways to Increase Metabolism:

* Never ever skip meals, especially breakfast.
* After lunch and dinner, take a brief walk (10 to 15 minutes).
* Do NOT eat LESS than 1200 calories per day.
* Eat 50 to 60% of your calories from carbohydrates.
* Train with weights at least 3 days a week.
* Perform some form of aerobic exercise most days of the week.
* Drink plenty of water.
* Get Plenty of Sleep.

Q: Can't spicy foods, such as chili peppers and curry, boost metabolism?

A: Yes — but unfortunately not enough to have an effect on weight loss. "Anything that increases your body temperature will temporarily raise your metabolic rate to a certain degree," Peeke says. But with spicy food, the increase is so small and short-lived that it doesn't have an impact that will show on the scale.



Q: What will happen to my metabolism if I lose weight?

A: As you lose weight, your RMR will slow down because you have less body mass to support. As a result, your body requires fewer calories to sustain its vital functions. Consequently, you won't need to eat as much to feel satisfied and to fuel your exercise. If you don't further modify your eating and exercise habits, you'll eventually hit a weight-loss plateau. To get past the plateau and continue shedding pounds, if that's your goal, consume fewer calories (without dropping too low) or increase the intensity or duration of your workouts.


Q: What about supplements and other products that promise to elevate metabolism and melt fat?

A: Don't believe them! No pill, patch or potion can magically raise your metabolism enough to help you lose weight, Peeke says. If you want a quick metabolic boost, you're better off hitting the gym or going for a brisk walk.

Boosting Your Metabolism


What is best for boosting your metabolism?

Nearly every time you pick up a magazine or newspaper these days, you see advertisements for products that supposedly raise your metabolism. Whether they're trying to sell you exercise equipment or food supplements, these ads claim that the products turn up the heat in your body so you burn more calories even when you're not exercising. Sounds logical enough, but is it true?
No question, in today's fitness world people are mad for everything metabolic. But what exactly is this thing called "metabolism" and how much do we really know about raising it? For example, what's better for the boost- weight training or aerobics? And is it true that eating hot chili peppers fires up your rate of fat combustion? Here are some burning questions about burning calories, plus some answers that are sure to surprise you.



What does "metabolism" mean?

Your metabolism is the sum of processes involving energy production- specifically, the chemical changes in your living cells by which energy is provided for your vital processes and activities and new material is assimilated. You can also think about your metabolism as the number of calories you're burning at any given moment, whether you're watching "Seinfeld" or pumping away on the StairMaster. Naturally, your metabolic rate- the rate of energy production and expenditure- will be higher when you're exercising than when you're crashed on the couch.



Will aerobic exercise raise your resting metabolic rate?

Several studies have failed to show any significant increase in resting metabolic rate due to aerobic workouts. One, for instance, found that men who jogged three times a week for 30 minutes at about 70 to 80 percent of their maximum heart rate did not increase their RMR. But they were certainly burning lots of calories on the run.



What is "afterburn"?

This term refers to the fact that your metabolic rate stays elevated after you do any kind of exercise. But the effect varies greatly depending on the type of workout, how hard and, to some extent, how long you just exercised.

In a recent study, Christopher Melby, Dr.P.H., associate professor of nutritional science at Colorado State University in Fort Collins, and his colleagues monitored seven men who did a killer, 90-minute weight workout- six sets of 10 exercises with very little rest between sets. Two hours after the workout, their metabolic rate was 11 percent higher than normal. When the subjects were measured the next morning, 15 hours after the workout, their metabolism was still elevated by a hefty 9 percent, which could translate into as much as 150 extra calories burned.

In another study, Melby cut the workout down to five sets of each exercise and let the subjects rest longer between sets. The subjects' post-exercise metabolic rate also was 11 percent elevated after two hours; but by the next morning, the increase had dropped to less than 5 percent. "What that indicates is, the post-exercise metabolic rate [of a weight-training workout] is a function of exercise intensity primarily, and to some extent duration," Melby says. The numbers in both studies were impressive, but the workouts were far longer and more strenuous than most people have the time, strength, stamina and motivation to maintain. Melby believes that the concept of afterburn gets too much hype. After all, the calories you burn post-exercise are negligible compared to what you burn during the workout.



Does aerobic exercise also give you an afterburn?

Yes, but it probably doesn't last as long as the weight-training afterburn- unless you work out intensely or do interval training. Most studies have found that mild to moderate exercise elevates metabolic rate for only a few minutes to a few hours. If, for instance, you jogged at 70 percent of your maximum heart rate for a half-hour, you probably wouldn't experience an afterburn for more than an hour. "For most people it'll be less than that," Melby says, burning no more than 15 to 30 calories. On the other hand, highly trained athletes who can run for, say, 90 minutes at 75 percent of their maximum heart rate, might have significant afterburn for several hours. The bottom line: Focus on burning calories during your workout, not after.

As for why strength training has more of an afterburn than moderate aerobic exercise, scientists aren't sure. One possible explanation, Melby says, is that anaerobic exercise, such as weight training, might elevate levels of the hormone epinephrine, which stimulates metabolic rate. Another theory: The repair of microscopic tissue damage caused by weight lifting might expend more energy.



What about aerobic interval training?

Go for it. Afterburn seems to depend mainly on workout intensity (duration doesn't have nearly as much effect). So if you do aerobics, interval training with alternate bursts of activity will get the best post-workout rise out of your metabolism. "If you did a number of short bouts, say on the treadmill or the bike, at 95 percent of your max, the afterburn would be significantly greater than if you used the same number of calories, or even more, doing one longer, lower-intensity bout at 50 or 70 percent of your max," says Melby. "Your metabolic rate would be elevated for a longer period at a higher magnitude."
So there is truth in advertising. Some kinds of equipment can rev up your body's calorie-burning rate; just be sure to do high-intensity interval training for the best metabolic results.



Is there any way to lower metabolism?

Yes. Go on a very low-calorie diet. Your body will sense that it is being starved and will fight back by slowing down. "If you don't get enough calories, your metabolic rate can go down profoundly, says Callaway. Severely anorexic women, he says, burn 30 to 40 percent fewer calories a day- sometimes 50 percent- than women who eat normally. (In other words, an anorexic who takes in only 200-300 calories a day uses it up much more slowly that her normal counterpart.) It's not clear how much of a calorie deficit triggers a drop in RMR- there's probably a continuum. But, says Callaway, you can get into trouble by consuming fewer calories than your RMR requires.



Can't you prevent your resting metabolism from dropping by exercising when you diet?

Not necessarily, especially if you're on a very low-calorie diet. Two late-breaking studies divided subjects into four groups: diet only, diet plus aerobic exercise, diet plus weight training, and diet plus aerobics and weight training. After many weeks, After many weeks, both studies found no significant weight loss among any of the groups. "When you add a lot of exercise on top of a marked caloric restriction, you're not going to prevent resting metabolic rate from falling," says Thomas A. Wadden, Ph.D., a psychology professor and director of the weight and eating disorders Program at the University of Pennsylvania School of Medicine in Philadelphia who conducted one of the studies.

That doesn't mean you should skip the exercise. To the contrary, you should diet and work out with moderation. In Wadden's study where the diet was severe- 128 obese people ate only 925 calories a day for the first 16 weeks and 1,500 daily for the next 32- there were no significant differences in weight or body composition changes among the groups. (Both calorie amounts were severe for this population.)

Feeling blue? Here's how to eat yourself better:

1 Eat foods that release energy slowly.
Your brain relies on a steady supply of glucose in the blood to make the body's feelgood chemicals. Make sure that you start the day with a wholegrain cereal like muesli or bran flakes, snack on fruit such as apples, pears and medium ripe bananas and include a small amount of pasta, sweet potatoes or rye bread with your main meals.

2 Avoid long stretches without food. This can lead to low blood sugar levels and irritability followed by the temptation to binge on sugary, fat-laden snacks, which eventually pile on the pounds and make you feel even worse in the long term.

3 Up your folic acid intake. This B vitamin is crucial for the production of the brain's 'happy' nerve transmitters so choose dark green leafy vegetables (such as broccoli and spinach), black-eyed beans and wholemeal bread. Choose foods fortified with folic acid such as Special K, Cornflakes, Marmite and Horlicks.

4 Eat turkey, salmon and yoghurt. All are rich in the protein building block called tryptophan. Levels of this brain chemical plummet when you feel down and need boosting to fight the blues. Skimmed milk and fromage frais are also good choices. These foods are rich in calcium and magnesium, minerals that help to soothe frayed nerves and encourage a good night's sleep.

5 Eat oily fish, cashews and pumpkin seeds three times a week. Tuna, salmon or sardines, cashews and flax and pumpkin seeds are packed with essential fats that are vital for the brain's efficient functioning. A lack of essential fats has been linked to depression, according to nutritionist Dr Anthony Leeds from London University.

6 Look out for vitamin C-rich guava. It comes tops in the list of fruits rich in vitamin C, another nutrient needed for making mood-enhancing serotonin. Strawberries (fresh or frozen), oranges, kiwi fruits and peppers are other good sources of vitamin C.

7 When down days are linked to hormonal changes, try evening primrose oil. The Women's Nutritional Advisory Service recommends this course of action. As a source of GLA, an essential fatty acid, evening primrose oil has been proven to dampen down mood swings, iron out irritability and help those with PMS to smile again.

8 Make sure you're getting enough iron. If you don't eat meat, check out other iron sources. A lack of this mighty mineral brings on the kind of tiredness and fuzzy thinking that can easily be mistaken for depression. Get stuck into oily fish like mackerel, the dark meat on chicken and turkey and vegetarian iron-rich foods like baked beans, fortified breakfast cereals and sesame and sunflower seeds.

9 Try some herbal helpers. Dr Ann Walker, a medical herbalist from Reading University, recommends trying St John's wort or 5-http which have both been proven to lift low moods. So convinced of its powers are German GPs that St John's wort is on prescription for those gripped by mild depression.

10 Up your water intake. You should aim to drink between 1.5 and 2 litres a day. Any less and you risk dehydration which Dr Susan Shirreffs, an expert on dehydration from the University of Aberdeen, says leads to emotional swings and headaches, which can be mistaken for mild depression. And talking of drinking, go easy on the booze. There is no surer way to wreck your emotional stability and spiral into depressive moods than regular binges.

Lol... i woke up at ard 7 am...
I was too sleepy last night le.. just wanted to lay down and rest a while... but once i lied down on my bed... i think my body lay limp there lol... like dead ger...

Anyway got up pretty early this morning... ard 7 am.. I went to jog lol... haa haa so hardworking hor... a bit bo bian cos last night ate too sinful le.. met up with three of my jc friends and we went to celebrate PeiLi's birthday by having a simple dinner at Cafe Cartel .. Lol i had Fish and chips... It was so so only bah i Dun find it really that nice.... I think the best fish and chips i had was at Paradize Centre... the Breaded chicken i think can't recall fish and chips or what was superb... value for money leh ard 6 bucks You get soup bread and meal and so big the serving... wahahahahahaa but i on diet.. lol not too good for me. =X

Ah just heard from my ex it shifted place le? haiz so pity lol....

Hmm.. anyway before i met with Pei Li they all Went to Bugis with my poly mates and walk ard Bugis.. They had a meal at V8 Movie Cafe... Bev's Chicken cutlet look so nice =D and it is cheaper than Cafe Cartel.. =\ I could just stare cos i was meeting PeiLi they all soon.... LOl was kinda hungry then =X

Hmm.. as for my Pract test I guess i did so so bah.... I dun know my marks but i think it was around a C cos i did some mistakes... haiz... I went all the way to Woodlands the bloody Guardian lor... haiz but luckily she gave us some medication names so i can check at home... if not sure die... our assignments got count marks one lor...I heard from AiLing... they so lucky the guy who serve them let them in on all the medication info unlike the very niao lady zzzz....but guy ma see ger cannot resist LOL =X

Bad News on Eye Disease

MONDAY, Sept. 13 (HealthDayNews) -- A combination treatment of vitamin A and the omega-3 fatty acid DHA (docosahexaenoic acid) do not slow the progression of retinitis pigmentosa.
The bad news comes in two articles in the latest issue of the Archives of Ophthalmology.

Rentinitis pigmentosa is a progressive eye disease that can cause vision loss. It affects about one in 4,000 people worldwide. Previous studies had found that taking vitamin A slows the decline in retinal function and vision loss.

Harvard Medical School researchers tracked 221 people with retinitis pigmentosa, aged 18 to 55, for four years. Some took 1,200 milligrams per day of DHA and others took a placebo. All the patients took 15,000 international units per day of vitamin A, according to the researchers.

The study found that, at four years, progression of the eye disease hadn't slowed in any of the patients.

In a separate analysis, the researchers did find an impact on patients who were not taking vitamin A before the study.

"Among patients not taking vitamin A prior to entry, those in the DHA+A group (30 patients) had a slower decline in (vision loss) than those in the control +A group (35 patients) over the first two years; these differences were not observed in years three and four of follow-up or among patients taking vitamin A prior to entry," the study authors wrote.

The study findings support "a previous recommendation that most adults with the typical forms of retinitis pigmentosa should continue to take 15,000 IU/d of vitamin A palmitate under medical supervision to slow the course of their condition," the authors concluded.

heh heh you might thinking it is weird i am blogging at such a weird hour!!
hee i had such great fun....
LOl thanks to SHi ling my hao jie mei =D Jio me go Equinox...

The place there is kinda classy and the pple there were more to the working class i guess.. Most importantly is it is on the 71st floor scenery there is quite awesome =D
The music was kinda soft at first but as the time goes by... it gets darker and the music louder and more awesome dance mUsic!!

Maybe the only thing is that i guess u have to create yr own dance platform? There isn't really a place for the pple to dance ard... But everything is COOL =D I met several interesting and fun pple... Mei Nu Mei Ying, Big eyes Zhi....wen is it LOL i forget x__X sorry.. The kai lun and his pretty friend... Lol and one guy looks like Ren Xian Qi but i think he looks better than ren xian Qi..yah his name is Yi Zhong ha haa ...

i chatted for some while with Mei Ying and Zhi Wen... LOl very nice pple to chat with ^^ the fries and potato wedges were nice =X I ordered Cinderella such a nice name right!!!
